They're plastic molded discs (some are rubbery, the rubbery ones tend to keep the taste of what you've been drinking) that fit in the wide mouth of the bottle and create a sort of a spout that makes it much easier to drink out of. They don't interfere with the bottle closing, so you just put it in the mouth and forget about it.
